The wagon wheel flanges rose over the ice compressed in the flangeway of the crossing, and further off the rails. Snow workers should be trained according to the new, further specified work instructions. Maintenance should be monitored in accordance with the management agreement.
Recommendation
Work instructions for turnout cleaning related to winter maintenance should be specified, with special attention paid to ice removal.
