L2022-03 Serious incident in Pori on 19 August 2022

The Safety Investigation Authority, Finland, has decided to initiate an investigation under section 2 of the Safety Investigation Act (525/2011) into a serious incident that occurred on 19 August 2022. During a training flight the rear door separated from a Diamond DA42 aircraft operated by Finnish Aviation Academy.

Airline Transport Pilot Juha-Pekka Keidasto is appointed as the Head of the Investigation Team, and aircraft mechanic Jukka Jylö, senior safety investigator Timo Naskali, as well as air traffic controller (retired) Toni Solatie are appointed as members. Chief Air Safety Investigator Janne Kotiranta will serve as the Investigator-in-Charge.
