Safety Investigation Authority Finland has decided to initiate an investigation under section 2 of the Safety Investigation Act (525/2011) into the serious incident that occurred at Helsinki Airport on 25 November 2021 when a Fokker 50 airplane, operated by the Amapola Flyg airline, experienced an engine failure during take-off.

Airline Transport Pilot Juha-Pekka Keidasto is appointed as head of the investigation team, with Air Traffic Controller (retired) Kari Alvi, Airline Transport Pilot Jani Holmberg and Aircraft Mechanic Mikko Raatikainen as members of the team. Chief Air Safety Investigator Janne Kotiranta will serve as the Investigator-in-Charge.
