The Safety Investigation Authority of Finland has decided after a preliminary examination to initiate a safety investigation into the death of a crew member onboard the Gibraltar flagged general cargo vessel M/S ANDANTE. The accident took place in the port of Kotka on September 30, 2011.

The AIB investigator captain Risto Repo was appointed as investigator-in-charge for the case and investigator Juha Sjölund was appointed as investigator.
