T2022-02 Decision to initiate an investigation into the death of an elderly client in a sheltered housing unit in South Savo on 26 November 2022

Under section 2 of the Safety Investigation Act (525/2011), the Safety Investigation Authority, Finland, has decided to initiate an investigation into an incident on 26 November 2022, in which an elderly client died due to a dangerous substance in a private sheltered housing unit in South Savo. 

Jaakko Niskala has been appointed as the Head of the Investigation Team, and Sari Yli-Kauhaluoma (organisational psychology), Riitta Flinck (customer and patient safety) as well as Ilpo Valta (safety investigation) have been appointed as its members. Chief Safety Investigator of social welfare and health care accidents Hanna Tiirinki will serve as the Investigator-in-Charge.
