T2023-01 Lack of a medication in dose dispensing in Satakunta in 2023

Under section 2, subsection 2 of the Safety Investigation Act (525/2011), the Safety Investigation Authority, Finland, will initiate an investigation into an incident that occurred in the wellbeing services county of Satakunta in February 2023, in which an elderly home care customer died. The investigation will focus on the dose-dispensing service of the pharmacy and more extensively, the processes related to medication safety.

Alpo Vuorio, MD, has been appointed as the Head of the Investigation Team, and Kari Ylönen, MA, Päivi Porkka, MD, and Jari Kotimäki, LLM, Master of Laws with court training, have been appointed as its members. Chief Safety Investigator of social welfare and health care accidents Hanna Tiirinki will serve as the Investigator-in-Charge.
