The airline had not carried out a formal and comprehensive safety assessment in accordance with its SMS.

Recommendation

Air Charter Scotland documents and implements criteria for the need for a safety assessment, and any time these criteria are met, responsible persons shall carry out a safety assessment that encompasses all activities related to the change prompting the assessment.

Air Charter Scotland (ACS) has introduced a process in which each flight operated by the ACS fleet is assessed using a risk management method as part of the flight planning process. The risk management method is designed to identify potential severe scenarios (including operations to destinations in cold climate conditions), and if such scenarios are identified during the planning phase, management conducts a more detailed assessment of potential hazards.
If risks are identified through the risk management method, the risk assessment is continued using a computer program to reduce and mitigate hazards. If the identified hazards cannot be fully mitigated, the matter is referred to the safety committee. In accordance with the ALARP principle, a decision is made either to proceed with the flight or to determine that it cannot be operated at an acceptable level of risk.
